MEDICATION PRESCRIPTION FORM is required to be filled out by the prescribing physician whenever a medication, including over the counter medication, is being brought to the school. A separate form is required for each medication that is brought into the school.
AUTHORIZATION FOR PRESCRIPTION MEDICATION ADMINISTRATION FORM is required to be completed by the student’s parent or guardian when a child brings any medication to school
AUTHORIZATION FOR STUDENT SELF-ADMINISTRATION IN SCHOOL FORM should be filled out if the child’s parent or guardian has agreed to allow the child to self administer medication while in school. The school nurse will review each application on an individual basis to determine if she feels it is safe for the child to self administer medication while in school. Guides and Assistants are unable to assist the medication administration except for epinephrine via auto injection, such as an Epi-Pen. The nurse is only available to administer medications from 8:30-11:30 while school is in session. To be able to take medication when the nurse is not available a student must have permission to self-administer medication from their parent or guardian and the school nurse.
MASSACHUSETTS ASTHMA ACTION PLAN is a form that needs to be completed annually by the student’s Pulmonologist or Primary Care Physician for all students who are asthmatic.
FOOD ALLERGY ACTION PLAN is a form that needs to be completed annually preferably by the student’s Allergist or their Primary Care Physician. Please complete one from for each allergen as each allergen may be treated differently.

We would like to take this opportunity to welcome you to the Thacher Parent Association.
We are looking for people to help us with a variety of programs and committees, all of which are designed to promote the wonderful community at Thacher. From showing off your cultural heritage at the Around-the-World Holiday Brunch, pitching in to pull weeds and plant flowers on the school grounds, carpooling, tying ice skates, flipping pancakes at the Pancake Breakfast, or just coming to the meetings to brainstorm and find new ways of making sure we can have a great environment for our kids to grow in.
We understand that in today's world it is increasingly difficult to find time to participate, but that only makes the need greater. Whatever your time schedule and interests, we need you. Even if it is just to come out, play Frisbee, fly a kite, and support the school at one of our picnics.
We will be launching a website soon just for the TPA where you can find out information about upcoming meetings, various committees, and how you can help. We hope that you will take advantage of this and make the upcoming school year sensational!
